Short Term Medical Insurance Providers

For individuals opting for one of the HealthBridge Short Term Medical Insurance membership plans, a variety of healthcare providers can be accessed through the PHCS Practitioner & Ancillary network. Utilizing one of these providers allows members with Short Term Medical insurance to enjoy substantial savings on healthcare services. This is because the fees for providers within the PHCS Practitioner & Ancillary network are capped at the contracted or discounted rates set by PHCS, known as the Maximum Allowable Expense (MAE).

In Nebraska, all practitioner, ancillary, and facility charges are reimbursed at 150% of the Medicare allowable rates. As of February 2021, the PHCS Practitioner and Ancillary Network is not longer applicable in this state.

PHCS Practitioner & Ancillary Network

All individuals enrolled in HealthBridge Short Term Medical Insurance plans have the flexibility to utilize the PHCS Practitioner and Ancillary network providers*, ensuring a wide range of high-quality care options. Information about the PHCS network is conveniently available on your HealthBridge ID card.

  While members can benefit from the extensive PHCS network, they also have the freedom to choose any healthcare provider. The network’s flexibility allows you to select the best care option for your needs, without being restricted to a specific provider network.
• Facility Charges:
  It is important to note that PHCS contracted rates do not apply to facility charges. For such expenses, the insurance plan covers up to 150% of the rates allowed by Medicare. In Nebraska, practitioner, ancillary, and facility charges are consistently reimbursed at 150% of Medicare allowable rates.
• PHCS Network Benefits:
  Choice: Access over 814,000 practitioners, making it the largest primary PPO in the nation. If you choose to see an out-of-network provider (OON), you'll receive less coverage. Out-of-network coverage is not available in the states of Massachusetts and Washington, and coverage varies in the state of Maryland. Please call VSP at 800-877-7195 before seeing an OON provider.

VSP continually assesses the doctor network to ensure adequate access for members. VSP's access standard is one doctor in a 10 mile radius urban/suburban and one doctor in a 25 mile radius for rural. VSP utilizes reports to analyze and determine the percentage of members that will have access to a doctor within a specified distance. VSP runs specific reports to determine if standards are being met and whether or not to apply appropriate interventions when gaps are identified. Not all provider locations offer all services. Eye exams may be performed by Independent Doctors of Optometry at a location adjacent to retail locations, as required by law. Please check with your VSP network doctor to find out if he or she provides the services you need. VSP contracted providers cannot discriminate against the hearing impaired, developmentally challenged and/or physically challenged. They must also allow full and equal access to covered services, including insureds with disabilities as required under the federal Americans with Disabilities Act of 1990 and Section 504 of the Rehabilitation Act of 1973.

VSP network doctors have the following certifications: Optometrists (Doctors of Optometry) are Therapeutic Pharmaceutical Agent (TPA) certified and Ophthalmologists are American Board of Ophthalmology (ABO) certified.
